Ultrasound as a Diagnostic Tool

Ultrasound offers several advantages over other available imaging tools such as live x-ray imaging (fluoroscopy), CT, and MRI. It is a readily available and cost-effective and can help evaluate tendons, muscles, ligaments, and nerves with a resolution that can surpasses MRI. Ultrasound can also be performed live, allowing for dynamic evaluation of ligaments and tendons, and it does not use radiation, so it is safe during pregnancy.
Ultrasound can be an effective tool for diagnosing problems in patients with difficult-to-evaluate conditions. For instance, to determine whether a tendon tear is the source of a patient’s shoulder pain, ultrasound is used to precisely inject anesthetic into the tendon sheath and if the patient feels significant improvement, it is an indication that that pain was originating from that tendon. Ultrasound can also help diagnose the source of painful movement-related snapping and popping sounds. By evaluating patients dynamically while they demonstrate their symptoms, we can identify things like unstable tendons and snapping calcifications.

Common applications of diagnostic musculoskeletal ultrasound include the:

  • Identification of tendon or ligament tears, compressed nerves, cysts, joint effusions and foreign objects.

  • Evaluation of painful popping and snapping sounds.

  • Diagnostic injections into joints and tendon sheaths or around nerves to determine potential pain generators.

Ultrasound-guided therapeutic procedures

Ultrasound allows us to precisely and safely deliver therapies to affected tissues. During ultrasound-guided injections or aspirations, we can directly visualize the needle approaching the target. Direct visualization confirms accurate injectate placement and offers a much greater margin of safety.

Applications for ultrasound-guided injections include:

  • Needle placement into joints for aspiration or injection, especially in patients with challenging anatomy, on blood thinners, or those in whom a non-guided injection has failed.

  • Injection into tendon sheaths or bursae.

  • Aspiration and injection of ganglion cysts.

  • Diagnostic or therapeutic nerve blocks, including carpal tunnel syndrome.

  • Percutaneous treatment of calcific tendonitis
